چکیده

Abstract Background and Aims The revised Chronic Kidney Disease Epidemiology Collaboration (CKD EPI) equation for estimated glomerular filtration rate (eGFR) reduced CKD prevalence among Asians, but few studies have evaluated its impact on healthcare utilization. We aimed to evaluate the of 2009-eGFRcr(ASR) [1] 2021-eGFRcr(AS) [2] equations risk hospitalization kidney disease multi-ethnic Asians. Method Retrospective cohort study adults who consulted at Singapore's largest cluster public institutions had a baseline serum creatinine performed in 2014. eGFR was calculated using both eGFRcr(ASR) categorized as G1 (≥90 ml/min/1.73 m2), G2 (60-89 mL/min/1.73 G3A (45-59 G3B (30-44 G4 (15-29 G5 (<15 m2). Electronic medical records were used identify demographic clinical information, outcome including acute injury, chronic disease, dialysis or transplant next three years until December 2017. Multi-variable logistic regression models compare disease. Model discrimination assessed area (AUC) under receiver operating characteristic (ROC) curve. Results Among 33007 Asian included study, median age 69.0 (interquartile range 63.0, 76.0). majority Chinese (79.5%; Indian 7.2%, Malay 10.2% other race 3.2%), female (53.3%), diabetes (56.4%) received statin therapy (71.6%) baseline. Hospitalization occurred 1915 (7.1% 26874 individuals) after excluding those with missing covariates (n = 5133). incidence increased worsening function (Table 1) regardless estimation used. After adjusting age, sex, ethnicity, comorbidities (cardiovascular diabetes, hypertension, cancer, systolic diastolic blood pressure, body mass index) medications (statin, ACE inhibitor, angiotensin receptor blocker, loop thiazide diuretic), categories independently associated 2). Compared G1, risks G2, G3A, incremental approximately 2, 6, 10 20 times, respectively, while 100 times that G1. AUCs 0.859 (95% CI: 0.849-0.869) model eGFRcr (ASR) 0.861 0.852-0.871) 2021-eGFRcr (AS) difference not significant (p 0.11). Conclusion Lower eGFRs ascertained by 2009 greater did significantly alter discrimination.

Comparison of risk prediction using the CKD-EPI equation and the MDRD study equation for estimated glomerular filtration rate.

CONTEXT The Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI) equation more accurately estimates glomerular filtration rate (GFR) than the Modification of Diet in Renal Disease (MDRD) Study equation using the same variables, especially at higher GFR, but definitive evidence of its risk implications in diverse settings is lacking. OBJECTIVE To evaluate risk implications of estimated ...

Evaluation of Glomerular Filtration Rate Estimating Formulas in Diabetic Patients with Chronic Kidney Disease

Objective: Measurement of glomerular filtration rate (GFR) is the best determinant in assessment of kidney function for diagnostic and therapeutic purposes. As extremely accurate methods of GFR measurement (i.e. Inulin clearance) are expensive and Time-consuming and due to limitations of the 24 hour urine collection method, some formula have been developed for the GFR measurement. Here we have ...

Estimating glomerular filtration rate using the chronic kidney disease-epidemiology collaboration creatinine equation: better risk predictions.

Serum creatinine is measured more than 280 million times annually in the United States, and more than 80% of clinical laboratories now report an estimated glomerular filtration rate (GFR) when serum creatinine is measured.1,2 The most commonly used equation is the Modification of Diet and Renal Disease (MDRD) Study equation. Recently, the Chronic Kidney Disease Epidemiology Collaboration (CKD-E...

Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI) Equation Pronouncedly Underestimates Glomerular Filtration Rate in Type 2 Diabetes

OBJECTIVE To evaluate the performance of the Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI) equation to estimate glomerular filtration rate (GFR) in type 2 diabetic patients with GFR >60 mL/min/1.73 m(2). RESEARCH DESIGN AND METHODS This was a cross-sectional study including 105 type 2 diabetic patients. GFR was measured by (51)Cr-EDTA method and estimated by the MDRD and CKD-EPI e...

Introduction of the CKD-EPI equation to estimate glomerular filtration rate in a Caucasian population.

BACKGROUND Chronic kidney disease (CKD) is defined as the presence of kidney damage, albuminuria or a reduction in glomerular filtration rate (GFR). A GFR <60 mL/min/1.73 m(2) alone is sufficient to diagnose CKD Stages III-V. Recently, the new chronic kidney disease epidemiology collaboration (CKD-EPI) equation was introduced. It has been suggested to result in higher estimated glomerular filtr...
